How do I sign up? 

1. Click on Registration Form tab  and fill out registration form.  All programs use the same registration form just check the desired program
 
2. Pay using our online PayPal option or send check payment to 10 Jeffrey Road Canton, MA 02021

* Please make sure you fill out the registration form!  Once registration form and payment are received you will receive a confirmation email from Extreme FH.

• Payment is due, in full, prior to start of program. 

 What is the refund policy?

• There are no refunds after the first week of program.
• Cancellations due to weather conditions are not rescheduled
• Any cancellations or delays due to weather will be posted online 1hr prior to event

 Do siblings receive a family discount?

• There is a 10% sibling discount for all Select Teams and Development programs.  There are no discounts for the Summer League program.

 Does Extreme supply goalie equipment?  Do I have to wear goggles?

• All Field Hockey equipment is the responsibility of the player.  Goggles are not mandatory, but strongly recommended.  Mouthguards and shinguards are mandatory!
