Options for Registration

 REGISTRATION OPTIONS:

    1.  Online Registration

       Take advantage of our online registration option.  Paypal can process credit

    card or echeck payments in a secure system.

    2.  Mail-in option

    Print, complete and mail registration form and payment to:

FYI....

  • Registration will close 2 weeks prior to the start of each session.  A late registration fee of $25.00 will apply if not received by the deadline.

  • You will recieve an email confirmation shortly after registering.
  • Payment is due in full prior to first session
  • Refunds will not be granted after the 1st session of any Extreme Field Hockey program

 

 

 Extreme Field Hockey will not grant refunds due to unforseen weather conditions Cancellations due to weather will not be rescheduled

  • Cancellations due to weather will be announced via email and under NEWS/FORMS section of website
